Referral pathways

My Aged Care and the National Disability Insurance Scheme (NDIS) are the first place to look for a list of allied health, community nursing and community service providers in your region.

All Allied Health and Community Service Provider referrals, including to Dementia Australia, for frail older people over 65 years is through My Aged Care. If patients do not meet the eligibility criteria a direct referral to Allied Health and Community Service Providers is required.

Allied Health and community service provider services, including access to Dementia Australia, for people under 65 years may be able to be accessed through the NDIS.

Memory Support Nurse – Stawell Regional Health.
