Practice nurse role in cognitive care
The role of the practice nurse is varied. However, it is likely that the practice nurse will provide care for people living with dementia and or the people providing them with support.
The practice nurse has a role in:
- Brain health promotion
- Recognising cognition change
- 'Assessing impact and adapting care' to meet individual needs within the context of cognitive change
- Including the support person
- Starting conversations about 'Planning for the future'
